Delayed root canal treatment of transplanted tooth for replacing missing tooth: a case report

Abstract
I. Introduction
Autotransplantation is a surgical movement of a tooth from its original location to another site in the mouth within the same person. If a tooth has deep severe caries, periodontal disease, trauma, or endodontic failure, it would be lost. Autotransplantation can be an alternative technique in various treatment options such as fixed or removable partial dentures, dental implants, or orthodontic space closure. If a donor tooth has complete root formation, it would be treated apicoectomy with MTA during surgery conventionally. However several studies reported that there is no difference between teeth treated apicoectomy with MTA and not be. Some successful cases also reported that time of endodontic treatment of the transplanted tooth had been variable. This case report describes a successful autotransplantation of a third molar to be treated no retrograde filling with MTA and delayed root canal treatment for replacing missing tooth.

II. Case Presentation
1. Sex/Age: M/29
2. Chief Complication(C.C): Referred from OMFS for root canal treatment of #36 (donor tooth)
3. Past Dental History(PDH): After #36 extraction, #28 transplantation to #36 extraction site at OMFS
4. Present Illness(P.I): mob(+), per(-), pal(-), gingival swelling
5. Impression: necrotic pulp of #36 (donor tooth)
6. Treatment Plan: Root canal therapy and crown treatment of #36 (donor tooth)
7. Treatment Progress: After autotransplantation, root canal treatment was started 2 months later
8. Follow up: A 17 months recall visit found the patient asymptomatic with no signs.

III. Conclusion
Autotransplantation has advantages such as short treatment time and less expensive than dental implants. Although it has some limitations including difficultness of tooth extraction and the possibility of side effects like root resorption, ankylosis, or periodontal problems, autotransplantation of a third molar to be treated no retrofilling and delayed root canal treatment may be considered as an effective treatment option for replacing missing tooth.
